i want to make labels for my cattys and forms. does anyone use labels with graphics on them or just plain white labels?

Does anyone have the current replacement form that they can upload for me? Every time I try to open the one on CC, Adobe Acrobat closes out on it before I get a chance to look at it. I just want to see what's all available on there.

Are we able to print out the Replacement order form and give it to the customer so they can mail in their own replacement part request. Or do we need to do it for them??
