agreements, conditions, exclusions and definitions, and this form does not amend, extend or alter the coverage so
described under the policy. This insurance does not pertain to the operation, use or maintenance of any car or other
motorized vehicle. Please contact the Finance department for coverage information.
REPORTING A CLAIM
In the event of a claim, write a report explaining in detail how, when and where the damage took place. Include
the name and address of the claimant. Report the claim as soon as possible, and include your name, address and
Consultant number when reporting a loss. Return the claim via mail, e-mail or fax to:

The insurance is $2 a month, and is automatically deducted from commission checks. It's nice to not have to worry about dropping spaghetti sauce on a host's white carpet. :)
