Here is a CANADIAN version HWC order form.

You can fill in info at the top. You can also fill in the tax rate and shipping amount.

I am assuming your ordering is the same as it is here for us in the U.S....online only.

I am only providing this form if you want to use it to collect orders and place an order for others. I am not saying you should or should not do this. I am posting this here since a Canadian Consultant requested that I create a Canadian version.
